Concerns regarding Ballots
“Ballot Duplication (by a candidate or stakeholder)”; “Neighborhood Council Board Endorsement of a Candidate or Slate of Candidates”; “Incorrect Ballots (which caused voters to vote either significantly more or less for a qualified or incorrect candidate); and “Electioneering by Candidate(s).”
 
An illegal “Sample Ballot With Recommendations” (photo attached) not produced by the City of Los Angeles was distributed near the polling place.  It shows seven candidates’ names printed in green with green check marks next to their names (including Maria Silva, Diane Valencia, Kathy Grubert and Ann Job).  Also, three candidates’ names are printed in red and one in black with a line through all four of their names; those four lost (Roy Smith, George Ortega, Donald Neal and Laurie Fadness).
 
Maria Silva and Kathy Grubert’s husband were in the parking lot during the election giving the illegal “Sample Ballot”s to Stakeholders going into the nearby polling place.
 
Many people who have seen the illegal “Sample Ballot” indicate that, because of its appearance due to having Board Seat categories, candidates’ names and check boxes next to the names, and because of the title, “Sample Ballot,” it appears to be an official government-produced document, or more specifically, by the Neighborhood Council, which is an agency of the City of Los Angeles.
 
During the election a copy of the illegal “Sample Ballot” was obtained by candidate Laurie Fadness and shown to Stephen Box, DONE’s Director of Communications 

Ms. Fadness also made Ann Job, the SNC Election Manager, aware of it.
 
Ann Job was the SNC’s Election Manager; she took no action to prevent the distribution of the illegal “Sample Ballot.”  Maria Silva (Public Relations Coordinator) and Kathy Grubert (Vice-President of Communications) have been two of the five SNC Executive Committee Members for two years, working closely with Ms. Job on the production of meeting agendas and other business.  Ms. Silva was the SNC’s Outreach Committee Chair for 1 ½ years until recently.  Diane Valencia, the SNC Vice-President of Administration, also should be held responsible for and benefitted from this.  The names of all four are on the illegal “Sample Ballot” in green with check marks next to their names.  They are four of the five current SNC Executive Committee Members.  It reasonably can be assumed that Kathy Grubert knew that her husband would be distributing the illegal “Sample Ballot.”  Ms. Silva and Ms. Grubert knowingly and repeatedly participated in distributing the illegal “Sample Ballot”s and Ms. Job, as the Election Chair, was ultimately responsible for it.  All four know or should have known better than to have distributed or allowed the illegal “Sample Ballot” to be distributed.
“Ms. Silva, Ms. Grubert, Ms. Job and Ms. Valencia should be disqualified; their elections should be nullified; they should be prevented from being sworn in or taking office as Sylmar Neighborhood Council Board Members; and they should be referred to the City Attorney’s Office for criminal prosecution.  If they are disqualified and prevented from taking office, the composition of the Board will change because there are candidates who received votes who then would fill those Board Seats.  This would allow legitimately elected candidates to become Board Members and serve the community.

When you bring additional fields into a conversion, Quickbase often finds inconsistencies. For example, say you're converting your Companies column into its own table. One company, Acme Corporation, has offices in New York, Dallas and Portland. So, when you add the City column to the conversion, Quickbase finds three different locations for Acme. A single value in the column you're converting can only match one value in any additional field. Quickbase needs you to clean up the extra cities before it can create your new table. To do so, you have one of two choices:

  • If you want to create three separate Acme records (Acme-New York, Acme-Dallas and Acme-Portland) click the Conform link at the top of the column.
  • If the dissimilar entries are mistakes (say Acme only has one office in New York and the other locations are data-entry errors) go back into your table and correct the inconsistencies—in this case, changing all locations to New York. Then try the conversion again.
